Pursuant to Article 90 of the Law on Social Insurance 2024, it is stipulated as follows:
Application for death benefits
1. The application for death benefits for relatives of people participating in social insurance or reserving social insurance payment period includes:
a) Social insurance book;
b) A copy of the death certificate or execution record or a copy of the death certificate or a copy of the decision of the Court declaring him/her dead;
c) Statement of a relative;
d) Original or copy of the record of the investigation of occupational accidents for cases of death due to occupational accidents; copy of the medical record for occupational disease treatment for cases of death due to occupational diseases;
dd) Minutes of assessment of the level of reduced working capacity of the Medical Assessment Council or a copy of the certificate of the level of severe disability that shows the conclusion of the Medical Assessment Council clearly stating the rate of reduced working capacity for relatives with reduced working capacity of 81% or more.

Accordingly, the application for death benefits for relatives of people participating in social insurance or reserving social insurance payment period includes:
- Social insurance book;
- Certificate of death or excerpt of the death certificate or a copy of the death certificate or a copy of the decision of the Court declaring death;
- Statement of relatives;
- Original or copy of the report on the investigation of occupational accidents for cases of death due to occupational accidents; copy of the medical record for occupational disease treatment for cases of death due to occupational diseases;
- Minutes of assessment of the level of reduced working capacity of the Medical Assessment Council or a copy of the certificate of the level of severe disability may show the conclusion of the Medical Assessment Council clearly stating the rate of reduced working capacity for relatives with reduced working capacity of 81% or more.
