Minister of Health Dao Hong Lan said that, implementing Resolution No. 72-NQ/TW of the Politburo on a number of breakthrough solutions to strengthen the protection, care and improvement of people's health; at the same time, implementing Decision No. 150/QD-TTg of the Prime Minister promulgating the Work Program for 2025, the Ministry of Health is completing the draft Decree regulating a number of special allowance regimes for civil servants, public employees, workers and armed forces in public health facilities.
Salary from level 2 when recruiting, minimum vocational allowance of 70%
Resolution 72-NQ/TW of the Politburo emphasizes the innovation of style, spirit, and attitude in serving patients in conjunction with improving professional capacity and medical ethics.
Accordingly, doctors, preventive medicine doctors and pharmacists will be paid from level 2 right after recruitment, instead of having to accumulate seniority as before. At the same time, the vocational preferential allowance is increased to a minimum of 70% for people directly working as professionals at commune-level health stations and preventive health facilities; 100% for employees working in remote, isolated, border, island areas and specific areas such as psychiatry, forensic medicine, emergency resuscitation, and pathology.
The Resolution also requires improving the working environment, reducing pressure on medical staff, enhancing medical ethics training, communication skills and strictly handling acts of abuse of medical staff. At the same time, prioritize the development of medical human resources in disadvantaged areas, implement projects to train doctors for commune health stations, improve the quality of specialized training and expand international cooperation in research, technology transfer and human resource training.
According to experts, this new policy is not only to change the salary or allowance figures, but is a breakthrough to create motivation and encourage the medical team, especially the grassroots and preventive medicine forces.
Adjusting 3 main allowance groups
According to the Ministry of Health, the Ministry will propose adjusting 3 main groups of allowances.
First, the regular allowance is changed to on-duty allowance, in accordance with the Law on Medical Examination and Treatment 2023, expanding the scope of beneficiaries to working departments 24/7 such as resuscitation, emergency, anesthesia, newborns, and psychiatry.
Second, surgical and procedures allowances are calculated at a rate of 5-20% of the basic salary (equivalent to 117,000-468,000 VND/cause), applied to doctors, nurses, technicians, nurses and support staff, accurately reflecting the actual labor intensity.
Third, epidemic prevention allowances become a permanent mechanism, covering both epidemic prevention and control activities, poisoning, medical disaster prevention and community emergencies. Forces directly participating such as drivers, communications, and logistics are also supported with accommodation and labor protection.
The Ministry of Health also increased support for grassroots health care, transferred monthly allowances for village and hamlet health care, increased to 0.6-1.0 times the basic salary, and the village midwife received 1.2 times, helping to improve the income of about 120,000 frontline employees.
In addition, the draft expands the beneficiaries of allowances to the armed forces, forensic examination, and social assistance facilities. All are calculated according to the basic salary of 2.34 million VND/month, ensuring synchronization with salary reform.

According to Minister Dao Hong Lan, in the draft, the Ministry of Health has proposed to adjust the allowances for on-duty, surgical, procedures, anti-epidemic allowances and food allowance support levels in the direction of inheriting Decision No. 73/2011/QD-TTg but updated on the basis of the new basic salary.
The Ministry of Health has calculated according to the corresponding adjustment level, ensuring compliance with reality and contributing to motivating and encouraging grassroots health workers to work with peace of mind.
