The patient, Mr. TTN (45 years old, from An Giang) was admitted to SIS Can Tho International General Hospital in critical condition, deep coma, very low GCS, only 4 points. The right pupil was dilated, weak reflexes; the left pupil was also showing signs of dilation, the prognosis was very poor.
Diagnostic imaging tests revealed a severe brain hemorrhage, with bleeding under the meninges on the right side and an unusual mass in the brain. The medical team decided to perform emergency surgery on the patient.
Specialist Doctor I Nguyen Nhut Thai - Deputy Head of General Surgery Department, SIS Can Tho International General Hospital - said that cerebral arteriovenous malformation (AVM) is a rare congenital condition, and when it ruptures, it can cause a high risk of death.
“In this patient’s case, we performed craniotomy to remove the hematoma, stop the bleeding, and remove the abnormal blood vessel. The vascular malformation, about 2cm in size, was removed during the surgery. The skull bone tissue is stored in a tissue bank, and is expected to be reimplanted within 3 months when the patient is completely stable,” said Dr. Thai.
After only 3 days of surgery, the patient was awake and responded very well to treatment. After 10 days, the patient was discharged from the hospital in stable health, fully recovered, to the joy of himself, his family and the medical team.
Doctors say this is a very positive result. Because cerebral vascular malformations are the leading cause of hemorrhagic stroke in young people, with a mortality rate of up to 90% of cases.
According to Dr. Thai, cerebral vascular malformations can be treated in many different ways, depending on the condition and size of the malformation. If the patient has signs such as epilepsy, convulsions, or severe headaches, a CT scan or MRI should be considered for timely detection and effective treatment.
