Saving a patient with a giant abdominal aortic aneurysm

Can Tho - On November 21, news from S.I.S Can Tho International General Hospital said the hospital had just successfully performed surgery on a patient with a complicated abdominal aortic aneurysm that was at risk of rupture.

The patient is Mr. V.T.S (51 years old, from An Giang). According to the patient, about 6 months ago, the patient felt contractions in his abdomen and often had a fever. The patient went to a large hospital in Ho Chi Minh City for examination and was diagnosed with a benign lipoma.

After a while, the patient felt more abdominal contractions, the contractions were long, large and raised like those of an 8-9 month pregnant woman, and could hear the beating sound.

On November 6, the patient went to S.I.S Can Tho International General Hospital for examination. Through ultrasound, doctors suspected that the patient had an abdominal aortic aneurysm in the subrenal segment. The patient underwent an abdominal CT scan, the results showed an abdominal aortic aneurysm in the subrenal segment, threatening to rupture, with a huge size of up to 70x90x131 mm, extending to the bifurcation of the iliac artery.

With the patient's condition very dangerous, doctors require the patient to be hospitalized for immediate treatment, otherwise the risk of aortic rupture is very high, which can lead to rapid death.

According to doctors, there are two methods of treating abdominal aortic aneurysms. One is to perform stent graft placement, which is a fairly optimal method; the other is the surgical method of replacing the aneurysm with an artificial graft, which costs less, but has higher post-operative risks.

Because the cost of stent graft placement exceeded the family's ability, the doctors considered surgically replacing the aneurysm with an artificial graft to save the patient's life.

The surgery lasted nearly 9 hours and was a great success. After 5 days of surgery, the patient recovered very well, blood circulation was good, and kidney function was guaranteed.

Specialist Doctor Duong Hai Minh - Department of General Surgery, SIS Can Tho International General Hospital - said that this case is very complicated, because the size of the aneurysm has spread to the level of the left kidney and the left renal artery is tightly attached to the aneurysm, so during surgery the possibility of renal artery damage is very high.

According to Dr. Minh, this is a difficult and specialized surgery that can only be performed in hospitals with doctors who are well-trained in thoracic vascular surgery.
