Dr. Duong Manh Chien, Department of Plastic Surgery, Central Tropical Diseases Hospital, said that the department has just received male patient T.V.A (65 years old, Hanoi) in a condition of congenital severe left eyelid collapse. According to medical history, right from birth, the patient's left eyelid had completely collapsed.
The cause is determined to be due to the seriously weakened function of the eyelid lift muscle, almost completely lost. In normal people, when opening the eyes, the eyelid lift muscle pulls the upper eyelids up, helping light enter the pupils. In this case, this machine almost does not work, causing the eyelids to fall low, obscuring most, even almost all of the pupils.
When coming for examination, the patient's left eyelid almost collapsed completely. The condition of limiting light into the eyes has lasted for many decades, forcing the patient to get used to seeing life through a narrow gap.
Living with eyelid drooping from a young age, the patient has self-formed a compensatory mechanism to look by contracting the forehead muscles, pulling the eyebrow and upper eyelid skin up, creating a small gap for light to enter the eye. As a result, vision is still maintained at a certain level.
According to Dr. Duong Manh Chien, this is a common compensation mechanism in cases of severe congenital eyelid collapse that has existed for a long time, helping to maintain vision but causing no small discomfort, affecting daily life.
After surgery, the patient's eyelash slit was widened, both eyelids achieved better symmetry, light into the eyes was more favorable, both improving vision function and improving facial aesthetics.
Dr. Chien said that eyelid surgery is not too complicated in terms of overall technique but requires very high accuracy. Depending on the case, doctors will choose methods such as shortening the weight of eyelid lift muscles, thread eyelid suspension or using forehead muscle strips. For this patient, due to very severe eyelid collapse and complete loss of eyelid lift muscle function, doctors have chosen forehead muscle strip reshaping technique to replace the role of eyelid lift muscles and restore eye opening function.
