While working with an industrial corrugated iron grinder, Mr. N.V.H (25 years old, Hanoi) was suddenly knocked down by a hydraulic grinder more than 1 cm wide, crushing 4 fingers at the same time, causing the fingers to almost break apart, leaving only a very thin bridge of skin.
When taken to the Central Hospital for Tropical Diseases, the fingers were all white, cold, sticky, and completely lostwatering - a sign of an extremely critical condition.
Immediately upon receiving the patient, the doctors immediately activated the emergency micro-surgical procedure.
Dr. Duong Manh Chien, Central Hospital for Tropical Diseases, said: This is a type of crushed injury that is especially severe, complex and much more difficult to treat than cases of severed amputation due to a sharp knife. The entire tendon, bone, blood vessel and nerve structure in the damaged area was severely damaged. In cases of limb fracture, the "golden time" only lasts from 6 to 12 hours; if intervention is delayed, the fingers will no longer be able to survive. Therefore, the patient was transferred directly to the operating room almost immediately, even without full test results, to race every minute.
The surgery lasted 12 hours, the surgical team had to meticulously remove all the damaged tissue, shorten the bones to create a healthy cut, and find blood vessels that could still be grafted. In one finger, the pulse was too long, so doctors forced a pulse in the leg to make a graft.
The blood vessels that need to be connected are only a few hundred micrometers in diameter, requiring absolute accuracy in each operation. Each finger must have a complete reconnection of arteries, veins, tendons and nerves separately. At the same time, the team prioritizes processing fingers that play an important role in hand function in advance to shorten the time of anemia and optimize the chance of survival for each finger.
Thanks to timely intervention and precise microredoring techniques, all 4 fingers of the patient have now become rosy and warm again, with good blood supply and clear vital signs. The patient was conscious and mentally stable.
Dr. Duong Manh Chien predicts that hand rehabilitation is positive if the patient strictly follows the treatment regimen and practices rehabilitation according to instructions.
