Dengue complications make patients in critical condition

A 25-year-old male patient in Bac Ninh suffered from dengue fever with complications of corporal compression, an extremely dangerous complication that can cause muscle necrosis.

Working as a carpenter in Bac Ninh, Mr. C.V.T (25 years old) had a delivery trip to Lang Son in early October. He did not know exactly when he was bitten by a mosquito. Five days later, he began to feel a sore throat, tired, then had a high fever of 3940°C with a severe headache.

His family took him to the local health station, where he was diagnosed with influenza B and given a fever-reducing epidemic. The next day, the headaches and abdominal pain increased, he continued to be examined at a private facility and was suspected of having digestive disorders. When his symptoms did not improve, he went to the provincial hospital. Test results showed a decreased platelets, CT scans detected deep images around the pancreatic head, with free fluid in the abdomen. Tests confirmed that he had dengue fever.

After three days of treatment at the provincial level, the condition did not improve. The left forearm and wings will have swelling, pain, blood clots and bleeding under the skin. The ultrasound detected a blood clot in his muscle on his left forearm, and he was transferred to the Central Hospital for Tropical Diseases.

At the Emergency Department, Dr. Le Son Viet said: The patient was admitted to the hospital with eyesodism, severe subcutaneous hemorrhage in the arm and left armpit, and decreased platelets. Software ultrasound shows blood clots in the forearms, swelling, tension, pain. Doctors diagnosed Mr. T with dengue fever with complications of left arm cavity compression syndrome.

Dr. Nguyen Hong Ha, a senior expert at the Central Hospital for Tropical Diseases, said: Conspirative compression syndrome is a phenomenon of increased pressure on soft tissue in closed cavities, causing blood to not circulate in the tissue. Initial symptoms are often edema, swelling or blood clotting. If the edges develop in a closed compartment, where there is little space for the software to expand, the compartment pressure will increase. When the pressure exceeds capillary pressure, cell exchange slows down or stops, leading to local anemia and increased edema. If treatment is delayed, the muscles can become necroscopic, causing irreversible damage to the muscles and nerves, leading to permanent loss of limb function, such as paralysis, loss of sensation, or having to amputate the limbs.

Immediately after being diagnosed, the patient was actively treated, fluid circulation control was controlled by the hour and compression pressure was reduced in the left forearm cavity. After a period of treatment, he has recovered well.

Doctor Le Son Viet recommends: Dengue fever can cause many dangerous complications, not only shock, internal bleeding or organ failure but can also cause muscle bleeding, blood clotting, compartment compression or nerve damage. Patients should absolutely not self- infuse or treat at home without medical monitoring.

When there are warning signs such as abdominal pain, frequent vomiting, sluggishness, unusual bleeding, pain in the limbs, the patient needs to immediately go to a medical facility with the capacity to treat dengue fever for timely treatment.

In parallel with treatment, disease prevention is still the most important measure. People need to proactively kill mosquitoes, kill larvae, sleep under a guise day and night, and remove items containing stagnant water around their homes. Currently, the dengue vaccine is a proactive measure to help reduce the risk of disease and serious complications.
