accompanied by umbilical cord disease and the decision to have surgery in the "golden time"
Having a second pregnancy, at the 35th week, Ms. N.T.H (29 years old) suffered from vaginal bleeding and pain in the lower abdomen, so she went to Vinmec Hai Phong Hospital for examination. Exploiting the medical history, BSCKI Pham Thi Yen said that the pregnant woman had an uterine fibroid tumor and had been treated with medicine. Due to warning signs of the risk of premature birth, Ms. H was admitted to the hospital.
At 6:40 p.m. on December 10, while being monitored in the inpatient room, the mother suddenly had a complete rupture of the amniotic fluid, her fetal heart rate decreased to 70-86 cycles/minute, the cervix opened 3 cm while the head was still very high. Doctor Yen discovered that Ms. H had umbilical cord disease (nortial cord disease). This is a very dangerous obstetric situation when the umbilical cord falls under the placenta, causing compression, reducing, and even completely stopping the blood and oxygen supply to the fetus.
Immediately, the emergency alert (Code Yellow) is activated. The entire team of obstetrics, anesthesia - resuscitation and neonatology quickly gathered. The mother was transferred directly to the operating room.
"We have to face many complications that can occur during surgery for pregnant women, such as the risk of uterine lumps, hemorrhage, and severe respiratory failure due to premature labor. If not operated on promptly during the "golden time", both mother and baby could fall into critical condition, even dying," said Dr. Yen.

At 6:50 p.m., the cesarean section was performed urgently. The 2.3 kg baby boy was born safely and was received by a team of neonatologists, given warmth, and provided with intensive respiratory support before being transferred to the neonatal intensive care unit (NICU) for close monitoring.
Coordinate with multiple specialties to save the mother's life
For mothers, the challenge has not stopped. In addition to umbilical cord disease, the mother also has a very large uterine fibroid tumor (12x12 cm), increasing the risk of severe postpartum hemorrhage. In this case, doctors had to ensure the fastest surgical speed to save the fetus and prepare a full plan to control the mother's bleeding.
The fibroids are located in the right cornea of the uterus, causing the uterus to become weak and throbbing, with a very high risk of uterine lumps and blood clots. We predict that we cannot remove the fibroid tumor immediately during the surgery, so we are ready to remove the uterus if there is uncontrolled bleeding, Dr. Yen shared.
As soon as she received information about the critical developments of the case, BSCKII Bui Thi Thu - Head of the Obstetrics and Gynecology Department - immediately arrived to directly coordinate with the surgical team.
The initial concerns of the doctors have come true: The mother's uterus bled heavily and could not be contracted, the amount of blood lost increased rapidly, threatening the mother's life. To save the patient from the risk of severe bang, the doctors were forced to decide to remove the uterus and keep the two extra parts to ensure maximum safety for the mother. After surgery, the mother was monitored in the resuscitation area for active resuscitation, stable blood flow, and no further bleeding.
"During the emergency obstetrics and gynecology rescue, we only had a few minutes to change the situation. The important thing is that the entire system has been operating synchronously, according to the correct procedures, without hesitation even for a minute, said Dr. Thu.
At Vinmec Hai Phong, maintaining the 24/7 emergency obstetrics and gynecology alarm system, closely coordinating continuously between the Obstetrics - Anesthesia - Neonatal Resuscitation Department has helped doctors successfully handle many dangerous situations such as postpartum thrombosis, stillbirth, severe preeclampsia, umbilical cord stones. The specialized treatment regimen for each case has shortened the surgical time - a decisive factor in helping to preserve the life of the fetus and mother.
In addition, the hospital regularly organizes training and simulation of obstetric emergency treatment to improve the reflex skills of medical staff. It is the careful preparation, serious investment in people and processes that has helped Vinmec Hai Phong become a trusted address for many domestic and foreign mothers.
