H hospitalized in a state of multiple injuries
On September 10, Binh Dan Hospital informed that the unit coordinated with the Con Dao Military Medical Center to successfully perform an emergency treatment to save the life of patient N.P.H (17 years old) with a belly injury and multiple organ damage after a traffic accident.
Patient H was hospitalized in a state of multiple injuries after being hit by a motorbike from behind and falling 4 meters in front. Although the patient was given 1 unit (350 ml) of settled red blood cell, the patient's condition continued to worsen.
On the morning of September 9, the patient had a rapid pulse, a decreased glycemic index (Hb), and was preparing for a diagnosis of ongoing endogenous hemorrhage. The patient is at risk of shock and blood loss if not operated on promptly.
That same morning, the hospital's crew arrived in Con Dao. After a direct consultation, the doctors decided that the patient needed surgery.
Through a close-up abdominal endoscopy, many damages were recorded, including a broken left kidney, a tear in the muscles of the left rib cage - enlargement, left pleural blood supply and ahimitat. The left kidney hole has a large blood clot, the left kidney rupture at the upper cuc and cannot be preserved at the lower cuc.
The patient had open surgery to suture the muscle tear into the abdominal wall, place the left pleural drain and remove the left kidney to stop the bleeding to save the patient's life. Doctors carefully assessed the remaining internal organs to ensure that there were no other injuries.
After removing all the blood clots, washing the abdomen and placing 2 drainage tubes, the doctors proceeded to close the abdomen and skin. The total amount of blood lost during the surgery was 1,500 ml.
After surgery, the patient was resuscitated and actively cared for by experienced anesthesiologists. On the first post-operative day, the patient had stable vital signs, a soft abdomen, and good drainage.
Improving the capacity of emergency surgery in areas far from land
Associate Professor, Dr. Tran Vinh Hung - Director of Binh Dan Hospital shared that this was a complicated case of closed abdominal injury causing endogenous bleeding, thanks to the coordination of a multidisciplinary doctor, a fully equipped operating room, and a blood bank, the patient was saved in time.
In addition, the unit also supports the development of infection control and the establishment of surgical procedures. The hospital will gradually build sustainable medical resources for the locality.
This surgery shows the need to improve the capacity of emergency surgery in areas far from the mainland. During bad weather, air transportation has potential risks.
Strengthening and coordinating with multidisciplinary doctors from central hospitals in Ho Chi Minh City to open up a direction for developing the outpatient emergency system right in Con Dao. Surgical cases can be performed on the spot to take advantage of the "golden time" for emergency care, minimizing risks for patients when having to be transported far away.
