Rescue helicopter rescues fisherman hit in the face by pulley, traumatic brain injury

On September 3, Military Hospital 175 of Ho Chi Minh City said that it had just coordinated to bring a fisherman with multiple injuries from Truong Sa Lon Island Clinic to the mainland safely.

The patient is Mr. N.V.H (born in 1974, from Gia Lai), a fisherman. Previously, on September 1, while pulling nets, he was hit in the face by a pulley, fell to the floor and fainted.

At 0:45 on September 2nd, the patient was taken to Truong Sa Lon Island Clinic in a condition with a head wound about 3 cm long, jagged edges, and open meninges. X-ray results showed that the patient had a skull fracture and multiple complex facial and maxillofacial fractures.

Doctors at Truong Sa Lon Island Clinic provided emergency care and remote consultation via Telemedicine system with Military Hospital 175. The patient was diagnosed with multiple injuries, including traumatic brain injury, monitoring open traumatic brain injury, fracture of right temporal-mandibular skull, monitoring intracranial hematoma; maxillofacial trauma with fracture of the left inferior jawbone and right cheekbone.

Recognizing that the patient was seriously ill, at risk of brain damage, respiratory failure and progressive bleeding, Military Hospital 175 proposed emergency transport by air to the mainland.

Complying with the order of the Ministry of National Defense, the Air Emergency Team of Military Hospital 175 led by Major Dinh Van Hong took off from Tan Son Nhat airport. At 11:15 pm on the same day, the flight crew landed and received the patient.

Doctors quickly assessed the condition, inserted endotracheal tubes to protect the airway, supported mechanical ventilation and maintained special care regimen throughout the flight. After that, the patient was taken to the mainland and landed safely at Military Hospital 175.

Lieutenant Colonel, Master, Doctor CKI Dinh Van Hong, Military Hospital 175 said that for patients with severe traumatic brain injury, the important thing in the transportation process is to maintain the target blood pressure, ensuring cerebral irrigation. In addition, complex facial and maxillofacial injuries put the patient at risk of progressive bleeding, which needs to be closely monitored throughout the flight.

At the Emergency Department, patients continue to be screened for lesions, undergoing in-depth tests and consultations to determine the next treatment plan.
