Sending a question to the Social Insurance agency, reader N asked: "I paid social insurance at the company for 5 years and then quit, have received social insurance and preserved unemployment insurance. After that, I went to work as a worker, the company paid social insurance for me according to the old social insurance book number.
After paying social insurance, I went to the social insurance agency to ask for the return of the social insurance book cover, but was told that the company was working to apply for re-issuance, when I returned to the company, the company asked me to go to the social insurance agency. I would like to ask, how should I apply for re-issuance?
Regarding this issue, Vietnam Social Security answers as follows:
Based on the provisions of point c, clause 1, Article 4 of the process of collecting social insurance, health insurance, and unemployment insurance; issuing social insurance books and health insurance cards issued together with Decision No. 366/QD-BHXH dated April 29, 26 of the Director of Vietnam Social Security;
To reissue the social insurance book, Mr. N declares the Application Form for registration and adjustment of social insurance and health insurance information (Form TK1-TS) and submits it to the employer or social insurance agency.
At the same time, for convenience in using and updating social insurance payment data in a timely manner, Mr. N can register to receive the electronic social insurance book in Clause 10, Section 1, Part A of the Registration Declaration, adjusting information for social insurance and health insurance participation (Form TK1-TS), the electronic social insurance book is returned to the electronic data management warehouse, VssID application, and the participants' email and is integrated with the electronic identification account (VNeID) at level 2, digital social insurance account (VssID) as prescribed in Article 3 of Circular No. 09/2026/TT-BTC dated February 3, 2026 of the Minister of Finance regulating the creation of electronic social insurance books and health insurance cards.
