The Government has just issued Decree 188/2025/ND-CP detailing and guiding the implementation of a number of articles of the Law on Health Insurance (HI).
Clause 5, Article 6 of Decree 188/2025/ND-CP stipulates the contribution rate of the group of subjects who self-pay health insurance according to Clause 5, Article 12 of the Law on Health Insurance. Accordingly, the contribution rate of 4.5% of the basic salary will remain the same, including:
People in households participating in health insurance under the form of households; People living and working, people being cared for and cared for in charitable and religious organizations and establishments; Employees who do not receive salary or temporarily suspend labor contracts during leave; People who do not fall under the cases specified in Points a, b and c of this Clause.
Household members specified in Point a, Clause 5, Article 12 of the Law on Health Insurance when participating in health insurance under the form of household in the fiscal year will have their contribution rate deducted as follows:
The first person contributes 4.5% of the basic salary, the second, third, and fourth person contribute 70%, 60%, 50% of the first person's contribution, and from the fifth person onwards, the first person's contribution will be 40%.
In addition, Decree 188/2025/ND-CP also details the contribution rate paid or paid by employers or employees or employees as follows:
The monthly payment level of the subject specified in Points a, c, d and e, Clause 1, Article 12 of the Law on Health Insurance is equal to 4.5% of the monthly salary used as the basis for compulsory social insurance contributions, of which the employer pays 2/3 and the employee pays 1/3.
Group paid by the Social Insurance agency: The contribution rate is 4.5% of pension or disability; 4.5% of basic salary or 4.5% of unemployment benefits, depending on the subject as prescribed in Clause 2, Article 12 of the Law on Health Insurance.
Group contributed by the state budget: The contribution rate is 4.5% of the basic salary.
For groups supported by the state budget: The contribution rate is 4.5% of the basic salary and is supported by the state depending on the subjects specifically specified in Clause 3, Article 12 of the Law on Health Insurance.
The Decree also stipulates the State budget support level, including: Supporting 100% of the health insurance contribution level for people from near-poor households residing in poor communes according to the Decision of the Prime Minister and other documents of competent authorities.
Support at least 70% of the health insurance contribution rate for subjects specified in Clause a, Point 4, Article 12 of the Law on Health Insurance; Support at least 70% of the health insurance contribution rate for subjects specified in Point g, Clause 4, Article 12 of the Law on Health Insurance. The support period is 36 months from the time the commune where the subject lives is no longer in an area with difficult and especially difficult socio-economic conditions.
Support at least 50% of the health insurance contribution rate for subjects specified in Point i, Clause 4, Article 12 of the Law on Health Insurance. The support period is 1 year from the date the subject is confirmed by the competent authority as a victim according to the provisions of the Law on Prevention and Combat of Human Trafficking;
Support at least 50% of the health insurance contribution rate for subjects specified in Points b, c, dd, e and h, Clause 4, Article 12 of the Law on Health Insurance; Support at least 30% of the health insurance contribution rate for subjects specified in Appendix d, Clause 4, Article 12 of the Law on Health Insurance and Clause 4, Article 5 of this Decree.
The Decree takes effect from August 15, 2025, except for the provisions in Clauses 2 and 3, Article 70 of this Decree.
