From July 15, the highest on-duty allowance for medical staff can be up to 917,500 VND for 24 hour shifts, applied in special departments and special areas of special grade and grade I hospitals on holidays and Tet.
Decree 192/2026/ND-CP of the Government officially takes effect, stipulating a new special allowance regime for medical staff. In which, it is noteworthy that the on-duty allowance level is adjusted in an upward direction, with the highest level up to 917,500 VND for a 24-hour shift.
According to regulations, for special-class and class I medical facilities, employees on duty 24/24 hours on working days are entitled to 320,000 VND/person/duty session. This level applies to special-class hospitals, class I hospitals and some specialized facilities such as the Central Institute of Forensic Psychiatry.
Currently, there are 6 special-class hospitals nationwide, including: Bach Mai Hospital, Viet Duc Friendship Hospital, Central Military Hospital 108 (Hanoi), Cho Ray Hospital (HCMC), Hue Central Hospital and Thai Nguyen Central Hospital. Many central and provincial hospitals belonging to the group of grade I hospitals are also applying this allowance level.
For grade II hospitals and some forensic units, the on-duty allowance is 255,000 VND/person/on-duty session. The remaining medical facilities, commune health stations, out-of-hospital emergency facilities, military-civilian medical clinics... are entitled to 185,000 VND/person/on-duty session. For nursing and nursing facilities for people with meritorious services and some public social assistance facilities, they are entitled to 70,000 VND/person/on-duty session.
The Decree also stipulates that the allowance level is calculated higher in some cases. If on duty in special departments and areas such as emergency resuscitation, intensive care, surgery - anesthesia resuscitation, anti-poison, stroke, organ transplantation, maternity room, care for premature babies..., the allowance level is multiplied by 1.5 times.
In case of duty on weekly holidays, the allowance is calculated as 1.3 times the normal level; if duty on holidays and Tet, the benefit level is 1.8 times.
Thus, a medical staff member on duty 24 hours at a special department or special area of a special or first-class hospital on holidays and Tet will receive an allowance of 325,000 VND × 1.5 × 1.8 = 877,500 VND. Adding 40,000 VND for meals/shift, the total support level reaches 917,500 VND/person/shift, the highest according to new regulations.
