Application for direct payment of health insurance

The Lao Dong Newspaper Legal Consulting Office replied:

Article 55, Decree 188/2025/ND-CP stipulates the dossier for requesting direct payment of health insurance as follows:

1. The document proposes direct payment according to Form No. 10 of the Appendix issued with this Decree.

2. The documents are photocopies including:

a) Health insurance card or health insurance code in cases where there is information about electronic health insurance cards and personal identification cards as prescribed in Point b, Clause 1, Article 37 of this Decree. In the case specified in Clause 5, Article 54 of this Decree, health insurance cards include cards with old benefits and cards with new benefits higher;

b) Hospital discharge paper or medical examination form or medical examination book of the medical examination and treatment requested for payment (photocopy with a hanging stamp of the medical examination and treatment facility);

c) Prescription (if any).

3. Invoices and cost statement.

4. In the case specified in Clause 6, Article 54 of this Decree, the dossier requesting direct payment, in addition to the provisions in Clauses 2 and 3 of this Article, also includes the following documents:

a) Filed documents for the designation of medical equipment for patients in cases of designation of medical equipment;

b) Confirmation form of shortage of drugs and medical equipment as prescribed in Form No. 11 of the Appendix issued with this Decree issued by medical examination and treatment facilities to patients.

As such, the dossier for requesting direct payment of health insurance is regulated as above.
