On March 9, regarding the reflection on the revision of semester exam scores at To Hien Thanh High School, the functional sector of Thanh Hoa province issued an inspection conclusion.

According to a report by the Thanh Hoa Provincial Department of Education and Training, the test results determined that when implementing the entry of mid-semester and end-semester I test scores for centralized exam subjects, 39 teachers of To Hien Thanh High School performed the entry - correction, deletion and re-entry of scores.
In the period from January 4 to January 16, there were 1,299 entries - corrections of scores belonging to 8 exam subjects concentrated in 29 classes. Among them, there were 566 increases in scores and 733 decreases in scores. The VnEdu system (online education management system) also recorded many entries performing the operation of deleting - entering scores.
From January 17 to January 19, after the school announced the review, there were 519 entries - corrections of scores for 8 subjects in 19 classes. Among them, there were 287 points increased and 232 points decreased.
Through verification, the above operations of teachers are all carried out in step 8 of the 12-step process issued by the school; taking place in many teachers and many subjects, mainly in the score entry stage. Regarding the deletion - entering operations, it is currently not possible to determine whether there is a score difference after deletion and re-entry.
Through random review and comparison of mid-semester and end-semester I test scores on the VnEdu system with actual test scores on students' papers (which have been publicly disclosed to students by the school), the inspection team found the results to be appropriate.
The inspection team also identified some errors of teachers stemming from mistakes in score columns, deviations in student order or not clearly understanding the rules for rounding scores. For the remaining cases, the inspection team did not have enough basis to determine subjective causes.
According to the teacher's explanation, the main errors arose during the score entry process due to incorrect score entry, misaligned view of the score column or misaligned student order.
Thanh Hoa Department of Education and Training also said that To Hien Thanh High School has issued documents regulating and guiding the management and use of electronic score books; inspection procedures. However, these documents do not specify; the organization of public dissemination, thorough understanding, training and implementation guidance to all officials, teachers, students and parents is not complete and not regular.
In addition, the school's internal inspection and supervision of score entry on the electronic score management system is still limited, not promptly detecting and correcting errors arising during the implementation process.
Previously, according to feedback, at the mid-semester I and late-semester I exams of the 2025-2026 school year, a Literature teacher of the school corrected 126 scores of students of grades 11C4, 11C9 and 12B3 when entering scores to the VnEdu system.
The scores were mainly revised in the direction of increasing from 1-3 points compared to the actual test and the results were initially announced by the school. Even, there were cases where students from 5-6 points were increased to 8 points, from 7 points increased to 8 points. In addition, some cases of students being reduced scores were also recorded.
After receiving feedback, To Hien Thanh High School reviewed and discovered that incorrect grade entry occurred in many other subjects, and at the same time requested professional teams to continue to inspect and rectify.
