The 13-month-old boy died while being transferred from Tay Ninh General Hospital to Ho Chi Minh City. The hospital's Board of Directors admitted errors in treatment and reviewed the disciplinary action against the two nurses.
The director of Tay Ninh General Hospital said that the child was hospitalized on the evening of August 9 due to measles with complications of pneumonia, and that at 12.8 doctors prescribed Adrenalin air solution but the nurse injected it directly. On the morning of August 13, the child had to use a ventilator and was transferred to Ho Chi Minh City Children's Hospital, but had cardiac arrest on the way and died the same day.
Immediately after receiving the information, the Minister of Health issued an urgent directive, assigning the Department of Medical Examination and Treatment Management to issue a document to the Tay Ninh Department of Health requesting handling of the incident.
According to the Department of Medical Examination and Treatment Management, this was a medical complication. Tay Ninh General Hospital must follow the procedures according to Articles 100 and 101 of this law to conclude whether there are or not technical errors, as a basis for resolving disputes (if any).
The Department also emphasized the establishment of a Professional Council to review the entire professional and care process from the time the child is hospitalized to the incident, especially in the stage of diagnosis and indication of adrenalin gasoline treatment according to Decision 3312/QD-BYT dated August 7, 2015 of the Ministry of Health.
The Ministry of Health requests the Department of Health and the leaders of Tay Ninh General Hospital to urgently contact, meet, and encourage the patient's family, and at the same time coordinate with the insurance agency and related units to propose a reasonable and reasonable solution. This is to ensure the rights of patients, while balancing the responsibilities of medical staff and hospitals.
To avoid false information, the Department of Medical Examination and Treatment Management requested the Department of Health to appoint a unit to speak and agree with Tay Ninh General Hospital in providing information to the press when requested. The conclusion of the Professional Council also needs to be announced soon.
In addition, this incident is a warning lesson about patient safety, the Ministry of Health requests the Department of Health and Tay Ninh General Hospital to learn from experience, strengthen professional processes and strictly implement regulations on quality improvement and prevention of medical incidents.
The document clearly stated: The Tay Ninh Department of Health must urgently implement the instructions, and at the same time send a detailed report and conclusion of the Professional Council to the Department of Medical Examination and Treatment Management before August 22, 2025.
