The patient is L.H.T (17 years old, Cai Rang district, Can Tho city), who had a traffic accident and was transferred to the emergency room at Hoan My Cuu Long Hospital in a state of rapid breathing, rapid heartbeat, sweating, abdominal pain, severe pain in the right flank and lower back, pale skin and mucous membranes, showing signs of severe hemorrhagic shock.

Through examination and necessary paraclinical tests, the results of abdominal computed tomography (MSCT) showed that the patient had a large right liver bruise and rupture, partial parenchymal defect extending to the liver surface, many bleeding vascular leakage points, and a large amount of intra-abdominal blood.
Doctors diagnosed the patient with liver rupture (grade IV) involving segments V, VI, VII, VIII, blunt abdominal trauma with internal bleeding and multiple injuries.
After a quick consultation, the doctors determined that the patient was losing blood and was in danger of dying, so they quickly performed intensive resuscitation, performed emergency surgery to preserve the ruptured liver, and performed a blood transfusion. After more than an hour, the surgery was successful and the patient was out of danger.

Currently, after surgery, the patient is awake, has no fever, pink mucous membranes, soft abdomen, no more bleeding, eats normally, vital signs are stable and has been discharged after 9 days of treatment.
Specialist Doctor Huynh Van Ut Chot - Department of Gastroenterology, Hoan My Cuu Long Hospital - said that liver rupture in closed abdominal trauma is very complicated, requiring correct diagnosis and appropriate treatment to avoid the risk of death due to hemorrhagic shock.
For this patient, the hemostasis was well managed, the liver was preserved at a very young age, combined with blood transfusion through the Cell Saver automatic blood filtration system to help limit blood loss and speed up recovery time. This is a modern automatic blood filtration system currently being used at Hoan My Cuu Long Hospital to help treat the patient's lost blood and re-transfuse it to them, minimizing the need for blood transfusion, especially in cases of emergency surgery where blood is not available.
Doctors recommend that accidents that are determined to have internal organ rupture or internal bleeding are very dangerous. If treatment is delayed or incorrect, the patient is at risk of immediate death. Therefore, when there are signs and symptoms such as abdominal pain, dizziness, nausea after a collision or fall, the patient should be taken to a hospital with full specialties and modern equipment as soon as possible for timely examination and treatment.
