According to the Ho Chi Minh City Department of Health, electronic health records are built on the basis of data connection from the Community Health Management Platform and electronic medical records at hospitals. Thanks to that, health check-up results are no longer individual medical records but are stored centrally, forming a continuous health data warehouse, helping people proactively monitor their health status, and at the same time support doctors in the process of diagnosis and treatment.
In the immediate future, the system has updated the data of people participating in the Universal Health Examination Program implemented by Ho Chi Minh City. After each periodic health check-up, information will continue to be supplemented, gradually completing electronic health records according to the life cycle of each citizen.
The City Department of Health encourages people to actively participate in periodic health check-ups according to invitations from local authorities or at qualified health check-up facilities. Each check-up not only helps detect diseases early but also supplements data, helping health records become more complete and accurate.
Currently, electronic health records on the City Digital Citizen application allow people to look up two main groups of information.

The first group is general information, including full name, personal identification number, date of birth, gender, occupation, address, phone number, blood type, health insurance card number, information for relatives to contact, allergy history, medical history, vaccination history and summary of recent health check-ups. These data help doctors quickly grasp necessary information when people come for examination.
The second group is health information, storing all health check-up results for each year. People can monitor physical indicators such as height, weight, BMI, blood pressure, pulse; clinical examination results of specialties; blood and urine tests, diagnostic imaging along with health classification conclusions, risk factors and doctor's recommendations. According to the Ho Chi Minh City Department of Health, people should regularly monitor conclusions and recommendations after each examination to proactively adjust lifestyle and prevent diseases.
Notably, from December 2026, when the Ho Chi Minh City Health Sector Data Warehouse is completed and put into operation, electronic health records will continue to be expanded. People can look up outpatient examination history, inpatient treatment, disease diagnosis, prescriptions, test results, diagnostic imaging, surgery and other treatment information. Data interconnection between medical facilities will help doctors quickly access medical history, limit the repetition of unnecessary tests, and improve the quality of continuous healthcare.
To use electronic health records, people only need to download the City Digital Citizen application on the App Store or Google Play, log in with the VNeID electronic identification account, then select the Health section and access the Electronic Health Record to track their information.
The Ho Chi Minh City Department of Health notes that electronic health records are only displayed to the owner after verification with a VNeID account. People should not share personal health information with unauthorized people and should continue to go to medical facilities for direct examination when there are abnormal signs.
According to the health sector, the formation of electronic health records for each citizen will contribute to building a proactive, continuous health care model and putting people at the center in the digital transformation process of the Ho Chi Minh City health sector.
