Severe pain in the left scrotum, the patient rushed to the medical facility for immediate examination

According to Master, Doctor Trinh Kien Cuong - Department of Andrology, Hanoi Andrology and Infertility Hospital - the patient felt severe testicular pain and was afraid of testicular torsion. The patient was rushed to the hospital for examination and timely surgery, so he was able to preserve maximum testicular function.
At the hospital, through clinical examination, the doctor found that the left testicle was hanging high, rotated, and painful when pressed. On ultrasound, there was an image of testicular torsion. The testicle, epididymis, and spermatic cord below the torsion site lost the blood supply signal on the Doppler spectrum. The patient was indicated for immediate emergency surgery to preserve maximum testicular function.
“From the time the patient entered the hospital until the surgery began, it was only about an hour. The examination process was very quick and we determined to operate as soon as possible. During the surgery, the doctors found that the spermatic cord was twisted three times and the testicles were purple. The surgery to untwist the spermatic cord was successful, and the testicles turned pink again. The doctors decided to preserve and fix both testicles to prevent recurrence,” added Dr. Cuong.
24 hours after surgery, the patient was examined and had a Doppler ultrasound showing that the blood vessels supplying the testicles had returned, both parenchyma had good blood supply, stable health, and no more pain.
Golden time to preserve testicular torsion
Master, Doctor Trinh Kien Cuong said: Testicular torsion is a phenomenon in which the spermatic cord is twisted around its axis, blocking the blood vessels that nourish the testicles, leading to edema and congestion.
If not promptly diagnosed and treated, the patient may face the risk of testicular necrosis or testicular atrophy due to lack of blood supply.
Testicular torsion is common in situations before or during puberty; when the weather is cold, leading to stimulation of the cremaster muscle to contract or there are structural abnormalities of the testicles.
Testicular torsion often occurs at night or early morning with symptoms of sudden, severe pain in the testicles, accompanied by scrotal pain, scrotal swelling, redness of the corresponding scrotal skin, lower abdominal pain and vomiting.
However, testicular torsion in young men may only cause pain but not much, causing many young men to be subjective, ignore the signs and often go to the hospital too late. The consequences of late treatment of testicular torsion will be very serious.
If the patient arrives at the hospital within 6 hours of the testicular torsion occurring, the chance of preserving and restoring testicular function is almost 90%. This is called the golden time for treating testicular torsion, starting from the time the pain appears.
