T (born in 1996) was admitted to Binh Duong Obstetrics and Pediatrics Hospital (Cach Mang Thang Tam street, Thu Dau Mot ward, Ho Chi Minh City) to give birth.
After examination, the doctor prescribed surgery, at this time the pregnant woman's health was normal. The pregnant woman entered the operating room at 11 pm, about 1 hour later, the doctor announced worsening developments. About 3 hours later, the doctor announced the pregnant woman's death.
On May 26, the hospital invited the family to work and announced that the cause of death of pregnant woman N.T. H. T. was amniotic fluid embolism during surgery. Through discussion, the family said that the explanation for the cause of the pregnant woman's death was not clear, the hospital showed signs of irresponsibility, and there was no emergency team to quickly handle the emergency situation. The family requested the hospital to provide a video of the surgical process for specialized agencies to re-evaluate.

On June 1, in an exchange with reporters, a representative of Binh Duong Obstetrics and Pediatrics Hospital confirmed that the pregnant woman came to the hospital to give birth at 10:50 PM on May 17. At about 11:15 PM, the pregnant woman was taken to the operating room. At 11:43 PM, when the uterine muscle was incised, the amniotic membrane was incised, the amniotic fluid was completely ruptured. At this moment, the patient had cyanosis convulsions, loss of consciousness, apnea, quickly falling into a coma, and heart dislocation.
Doctors diagnosed the pregnant woman with amniotic fluid embolism while performing emergency fetal removal surgery. Immediately, the team performed simultaneous treatment, performed a cesarean section to give birth to a baby girl and provided emergency care for the pregnant woman.
The pregnant woman then continued to progress badly, the hospital issued a red alert and contacted the emergency department of Binh Duong General Hospital. The pregnant woman continued to be treated until 2:50 am on May 18, but did not survive.
Binh Duong Obstetrics and Pediatrics Hospital explained that the pregnant woman had amniotic fluid embolism during the fetal removal surgery, leading to complications such as: acute cardiovascular collapse, acute pulmonary edema, respiratory failure, blood clotting disorder, circulatory arrest during the prenatal surgery. This is a very dangerous obstetric complication, with a high mortality rate. This complication cannot be predicted and cannot be prevented.
The doctors have made maximum professional measures, with high responsibility for handling. When such an incident occurred, the doctors themselves were also very sad" - the hospital representative said.
It is known that the baby girl was born prematurely at 33.5 weeks, weighing 2.05 kg. Currently, the newborn baby is being cared for at the hospital, and the baby's health is stable.
Binh Duong Obstetrics and Pediatrics Hospital added that after the incident occurred, the hospital's professional council reviewed and learned from experience, and on May 27, it reported to the Ho Chi Minh City Department of Health.
A leader of the Ho Chi Minh City Department of Health said that he had received a report from Binh Duong Obstetrics and Pediatrics Hospital. Currently, the Department of Health has directed this hospital to establish a professional council to re-evaluate the entire process of receiving, treating, and performing surgery for pregnant woman N.T. H. T. After receiving the council's report, the Ho Chi Minh City Department of Health will re-evaluate and will inform the patient's family and press agencies of the results.
