On February 13, Quang Tri Provincial General Hospital said that patient H.T. P (21 years old, residing in Huong Phung commune, Quang Tri province) was transferred from a lower level to the upper level in a state of high fever, fatigue, and severe abdominal pain in the right hip. Through clinical examination, tests and diagnostic imaging, doctors determined that the pregnant woman had sepsis originating from the urinary tract, caused by urethral stenosis on both sides related to pregnancy compressing, causing hydronephrosis.
Recognizing this as a complex case, Quang Tri Provincial General Hospital urgently organized a multi-specialized consultation between the General Internal Medicine Department, General Surgery Department and Obstetrics Department to unify an optimal treatment regimen.
In addition to sepsis, the pregnant woman was also pregnant for the first time, her buttocks lacked leg style, she was in labor, and the fetus was slow to grow in the uterus. Doctors assessed that if she gave birth naturally, there was a high risk of fetal trauma, fetal dysfunction, and head and back disease.
Based on the consultation, the hospital agreed on a combined treatment plan. The Department of General Surgery proceeded to place JJ ureteral tubes on both sides to resolve the water retention in the renal pelvis and control the source of infection. The Department of Obstetrics and Gynecology closely monitored the disease progression, choosing the appropriate time for obstetric intervention to ensure the survival of both mother and fetus.
Thanks to the close, timely coordination and high sense of responsibility of the medical team, the pregnant woman overcame the critical stage and was safely performed a cesarean section. Currently, the mother's health is stable, and the baby was born healthy.
Through this case, doctors recommend that pregnant women need to have regular prenatal check-ups, especially in the last 3 months of pregnancy, to detect early kidney water retention. When there are symptoms such as fever, back pain, painful urination or other abnormal signs, it is necessary to go to a medical facility immediately for examination and timely treatment, avoiding dangerous complications.
