The Ministry of Education and Training announced the draft Circular Promulgating the Regulations on diplomas and certificates of the national education system to collect comments.
In particular, the re-issuance of diplomas and certificates is clearly regulated in Article 14 of the draft Circular. Specifically as follows:
Article 14. Re-issued degrees and certificates:
1. In case the issued certificate or certificate is found to be misspelled due to an error by the agency issuing the certificate or certificate, the agency that issued the certificate or the agency managing the original book has the authority and responsibility to reissue the certificate or certificate.
2. Procedures for reissuing degrees and certificates are as follows:
a) The person requesting to reissue the certificate or certificate shall provide the information requesting the correction on the online public service portal or send it directly or via postal service to the competent authority reissuing the certificate or certificate.
Requirements include:
The application for reissuance of a certificate or certificate on the form has information about: Personal identification number; number of the certificate or certificate; number in the certificate or certificate issuance book; date of issuance of the certificate or certificate; certificate or certificate for reissuance (if it is a certificate or paper certificate); documents proving that the agency issuing the certificate or certificate has written the wrong certificate or certificate or a link to the database to extract documents proving that the agency issuing the certificate or certificate has written the wrong certificate or certificate;
b) Within 03 working days from the date of receiving a complete and valid dossier, the competent authority reissuing the certificate shall consider deciding on reissuing the certificate; if not reissuing it, it must respond in writing and state the reasons;
c) In case the diploma and certificate sample at the time of granting the diploma and certificate has been changed, the agency issuing the diploma and certificate will use the current diploma and certificate sample to reissue the diploma and certificate.
3. Decision to reissue degrees and certificates Decision to revise degrees and certificates includes the following main contents:
a) full name, coffin, name; date of birth of the person with a diploma or certificate (according to the degree or certificate reissued);
b) Reasons for re-issuance;
c) Name, number, date of year of issuance, full name of the person granted the issued diploma or certificate with a fault that must be revoked or canceled;
d) The effectiveness and responsibility for implementing the decision. d) Information on instructions for reviewing the reissued diplomas and certificates.
4. The reissued certificate and certificate has the words "PORT Here" right under the name of the certificate and certificate. The date of signing the certificate is the date of reissuance of the certificate.
5. Update the database of diplomas and certificates in case of reissuance When reissuing diplomas and certificates, the agency reissuing the diplomas and certificates must update information about the degraded diplomas and certificates in the database of diplomas and certificates, and at the same time update the status of revocation or cancellation of the issued diplomas and certificates with errors.
