The health insurance (HI) contribution rate in 2026 for self-payers and students will not change compared to 2025. Notably, the group of HSSVs will receive a minimum support from the state budget from 30% to 50% from July 1, 2025.
The above information was clearly stated by the Social Insurance (SI) of Vietnam in Official Letter No. 1766/SI-QLT sent to the People's Council of Can Tho City, in response to the petition of voters of Chau Thanh A District (now Can Tho City), Hau Giang Province on the health insurance contribution level.
Previously, at the meeting with voters before the mid-2025 session, many voters reported that the health insurance contribution level has continuously increased in 2023-2024 due to adjusting the basic salary. In particular, the 2024 Law on Health Insurance stipulates that from 2025, the maximum contribution for voluntary participants will increase to 6% of the reference level, compared to 4.5% previously. According to the reflection, using the basic salary as the basis for health insurance payment is unreasonable for rural people - a group without a fixed salary and unstable income, easily affected by fluctuations in agricultural product prices and input costs for production. Voters recommend having a separate health insurance policy for households and social housing in 2025 and the following years.
Responding to the petition, Vietnam Social Insurance said: According to the provisions of Clause 11, Article 1 of the Law amending and supplementing a number of articles of the Law on Health Insurance 2024 and Decree No. 188/2025/ND-CP of the Government, the health insurance contribution rate of the voluntary group continues to apply 4.5% of the basic salary. In case the whole household participates in health insurance in the fiscal year, the members will be gradually deducted at a rate of 70 to 40% compared to the first contributor.
For the group of HSSV and people from farming, forestry, fishery and forestry households with an average living standard, the state budget still supports the minimum health insurance contribution level. Specifically, HSSV will be supported from 30 to 50% from July 1, 2025; the remaining group will be supported with at least 30%.
Vietnam Social Insurance affirmed that the health insurance contribution rate in 2026 will remain the same as in 2025. At the same time, this agency will receive voters' opinions and will synthesize and propose to competent authorities to consider increasing the support level from the budget to create conditions for low-income people to participate in health insurance, contributing to ensuring social security policies.
