In particular, the dossier for requesting a copy of the certificate from the original book is clearly regulated in Clause 4, Article 17 of the draft Circular. Specifically as follows:
4. The dossier for requesting a copy of the diploma or certificate from the original book includes:
a) The application for a copy of the diploma or certificate, on the Application form, there is information about: Personal identification number; number of the diploma or certificate (if any); number in the certificate or certificate issuance book; date of issuance of the diploma or certificate;
b) In case the person requesting a copy of the certificate from the original book is the person specified in Point b, Clause 3 of this Article, he/she must present the authorization letter (for the authorized person), the person specified in Point c, Clause 3 of this Article must present the documents or a link to extract information from the database of the competent authority proving the relationship with the person granted the original copy of the certificate;
c) In case the person requesting a copy of the certificate from the original book sends the request by post, a certified copy of the documents specified in Point a and b of this Clause must be sent with 01 (one) envelope with a stamp clearly stating the recipient's full name and address to the competent authority issuing a copy of the certificate from the original book.
In addition, Clause 5, Article 17 of the draft Circular also stipulates the procedure for granting copies of diplomas and certificates from the original book. Specifically:
a) The person requesting to issue a copy of the diploma or certificate providing information on the online public service portal or submit it directly or send it via post 01 (one) of the documents as prescribed in Clause 4 of this Article to the competent authority issuing a copy of the diploma or certificate from the original book;
b) On the day of receiving the request for a copy or on the following working day, if receiving the request after 03/30 (three) afternoons, the competent authority issuing a copy of the certificate from the original book must issue a copy to the person requesting the request.
In case the request for a copy of the original book is sent via the postal service, the deadline is determined from the time the agency receiving the request for a copy receives a complete and valid dossier according to the arrival postmark;
c) In case the request to issue copies of multiple original books at the same time is required to have many copies, the content of the certificate is difficult to check and compare, and the agency receiving the request for a copy cannot meet the deadline specified in Point b of this Clause, the deadline for granting a copy shall be extended by no more than 02 (two) working days or longer as agreed in writing with the person requesting a copy.
